Slated for another elbow surgery

Reds president of baseball operations Nick Krall said Friday that Greene has been diagnosed with a partial UCL tear in his right elbow and will undergo surgery next week, Brian Giesenschlag of Reds Live reports.

Analysis

The exact nature of the procedure will not be known until the operation is performed. Greene previously had Tommy John surgery in 2019 and underwent surgery in March to have bone chips removed from his elbow. Even if Greene doesn't require another full Tommy John repair, he will be in danger of missing a large portion of the 2027 season. The 27-year-old made only five starts this year after being limited to 19 starts in 2025.

Reds manager Terry Francona said Thursday that there's "probably a slim chance" Greene (elbow) pitches again this season, Charlie Goldsmith of Fox 19 Now Cincinnati reports.

Analysis

Greene felt discomfort in his right elbow during a bullpen session Tuesday and was placed on the 15-day injured list Wednesday. He will receive a second opinion from Dr. Neal ElAttrache in Los Angeles on Thursday before more is known about the hurler's long-term prognosis. Greene didn't make his 2026 debut until early July following surgery in March to have bone chips removed from his right elbow, and he also had Tommy John surgery back in 2019. It's unclear whether another operation is possible, but even if Greene is prescribed rest, he can be dropped in redraft leagues.

Analysis

Greene felt discomfort in his throwing elbow after Tuesday's side session, which prompted the Reds to place him on the 15-day injured list Wednesday. He opened the year on the IL after having bone chips removed from his right elbow in March and didn't make his 2026 debut until early July. Analysis

Greene's 2026 debut with the Reds was delayed until July 4 after he required surgery in mid-March to remove bone chips from his right elbow. After posting a 6.83 ERA, 1.37 WHIP and 33:9 K:BB in 27.2 innings over his first five starts, Greene has been shut down yet again due to an elbow issue. He'll likely be sent in for further testing before the Reds have a better idea of how much time he'll miss, but it's reasonable to expend the right-hander to be out beyond the 15-day minimum. Cincinnati recalled reliever Luis Mey from Triple-A Louisville in a corresponding move but will need to fill Greene's spot in the rotation later in the week.
